TYPE OF SERVICE:
Automatic controlled Outdoor Lighting, dusk to dawn photo-cell control. All lamps connected ahead of consumer meter.

 

CONDITION OF SERVICE:
Completing and signing (by electronic submission) this form is an application for service and a binding agreement. I acknowledge that I am authorized to request this service and authorized to add charges to the electric bill for the account listed below.

 

The applicant desires to have installed on their premises a photo-electric controlled luminaire equipped with a {see form below} watt mercury vapor lamp. All fixtures, poles and wires will remain the property of the District.

 

The applicant agrees to allow District personnel to enter their premises for the purpose of maintaining or removing said light or lights and to maintain District property and equipment at any time required by the District to perform such work.

 

The District is obligated to maintain, service and replace the bulb(s) and related parts only during normal working hours, namely:  7:30 am to 4:00 pm on week days except in case of emergency.  If the consumer requires service during times other than normal working hours, the customer agrees to pay for such services.

 

SERVICE CHARGES
The automatic controlled light fixture is to be connected ahead of the consumer’s electric meter. Monthly and annual charges for the light shall be at current rates specified in

and annual charges for the light shall be at current rates specified in Rate Schedule ‘L’, and subject to change by action of the Board of Directors. This is not a fixed cost contract.

 

TERMS OF SERVICE:
In consideration of the installation of a light(s), the applicant agrees to use and pay for same for a term of
five (5) years from date of installation and thereafter until this agreement is terminated upon notice by either party to this agreement. If this agreement is terminated prior to five (5) years from date shown below, then the customer agrees to pay the balance of the monthly charges. These charges may be added to the applicant’s electric bill and subject to disconnection of service for non-payment. After the five (5) year obligation has been fulfilled the applicant may purchase the light for $60.00. In doing so the customer understands the requirement to provide for the maintenance and pay the energy required to operate the light.

 

Authorization is hereby granted to install {see form below} (number) light(s) on poles or structures approved by a District representative, at my property listed on the Service Agreement below:
